Peran Uvula bifida terhadap pertumbuhan dan perkembangan bicara anak

Latar belakang: uvula bifida adalah kelainan bawaan rongga mulut yang berpotensi menyebabkan gangguan pada proses menelan dan gangguan bicara.
Kasus: anak laki-laki usia 4 tahun BB 11kg datang dengan keluhan utama BB sulit naik sejak usia 1 tahun dan bicara kurang jelas. Riwayat sering tersedak dan sulit menghabiskan porsi makanannya. Penyakit yang sering ditemukan batuk pilek. Sudah dilakukan pemeriksaan evaluasi kemungkinan TBC dan ISK dan hasil negative. Pemeriksaan fisik ditemukan uvula bifida derajat 1 dan pemeriksaan lain dalam batas normal. Tatalaksana konservatif berupa edukasi cara pemberian makan sedikit tapi sering, mengunyah perlahan-lahan, konsistensi makanan cair dan menghindari makanan pedas. Hindari bicara saat makan dan suplementasi Fe, Zinc dan multivitamin. Orang tua diharapkan selalu mengkoreksi bicara yang tidak jelas, mengatur pernafasan saat bicara dan menghilangkan kebiasaan yang membuat anak takut misalnya suara mengeras, memarahi. Pada kunjungan selanjutnya didapatkan perbaikan yang cukup bermakna dalam BB dan kemampuan bicara anak.
Kesimpulan: pada kasus malnutrisi perlu dicari masalah pemasukan dan pengeluaran makanan salah satunya yang sering terlewatkan adalah kelainan rongga mulut misalnya uvula bifida. Terapi konservatif dilakukan pada uvula bifida grade 1.
